However, there are some of the important requirements that are required while opting for MAT course are - one identification proof like State Identification Card or Driving License, a signed copy of an affidavit of lawful presence stating citizenship.

The MAT course has to be offered by a licensed Nursed who should either be RN or LPN, successful completion of 12 hours training, scoring a minimum of 85% on all the tests and has to pass the mandatory OSBI background check.
